RESUMO

O presente artigo tem por finalidade apresentar um caso clínico da instalação de um implante imediato em área estética com enxerto de tecido conjuntivo para a manutenção da estética peri-implantar. Paciente do sexo feminino, 52 anos, apresentou-se com o elemento dentário 11 fraturado. Ao exame clínico indicou-se a exodontia e a instalação de um implante imediato. Foram solicitados à paciente uma tomografia computadorizada e radiografia periapical para avaliar os tecidos moles e duros e realizar o melhor planejamento para o caso em questão. Após a remoção do resto radicular, o implante foi instalado imediatamente com o enxerto ósseo e o enxerto de tecido conjuntivo proveniente do palato, com prótese provisória imediata, respeitando todos os princípios dos espaços crítico e subcrítico. Após seis meses, observou-se a regeneração da área gengival ao redor do implante com a manutenção da espessura gengival vestibular necessária para confeccionar uma coroa de porcelana. Pode-se concluir que os provisórios implantossuportados são fundamentais para o adequado condicionamento dos tecidos moles peri-implantares. A literatura confirma que cada caso deve ser avaliado individualmente, considerando alguns critérios na definição do plano de tratamento, como a linha do sorriso do paciente, o biótipo gengival e o nível dos tecidos moles, para que sejam eliminados procedimentos que possam levar à falha estética e, consequentemente, atinjam as expectativas do paciente.

Immediate dental implant in the aesthetic zone with connective tissue graft and immediate provisional restoration: a 6-month case report

 

ABSTRACT

This article aims to present a clinical case of the installation of an immediate implant in an aesthetic area with a connective tissue graft, aiming to maintain the peri-implant aesthetics. A 52-year-old female patient presented with a fractured tooth #11. Upon clinical examination, the final indication was for extraction and the installation of an immediate implant. Then, CBCT and periapical images were requested to evaluate the soft and hard tissues and to provide the best treatment planning. After removing the root fragments, the implant was immediately placed, with a bone graft and a connective tissue graft from the palate, and the immediate provisional prosthesis, respecting all the principles of critical and subcritical spaces. After six months, satisfactory regeneration was seen at the gingival area around the implant, as well as the maintenance of the buccal gingival thickness necessary for a porcelain crown. It can be concluded that the provisional implant-supported prosthesis plays a fundamental role in adequate soft tissue conditioning. The literature confirms that each case must be evaluated individually, considering some criteria in the definition of the treatment planning, such as the patient’s smile line, the gingival biotype, and the level of the soft tissues, so that procedures that may lead to unaesthetic defects are eliminated. Consequently, the treatment can meet the patient’s expectations.
